Rural health care; Alabama Health Command established to designate an AI-assisted health platform in rural communities
HB280 (Alabama, introduced 2026-01-20 and pending in the House Committee on Ways and Means General Fund) has not been enacted. If enacted, it would create the Alabama Health Command, an independent agency led by a Governor-appointed director with knowledge of AI-based technology platforms. The agency would designate an Alabama-headquartered business to develop a uniform, interoperable, virtual AI-assisted health platform for consumer-initiated health management services in rural communities. The platform must be able to use a patented avatar-based virtual encounter system. Health management services include wellness education, scheduling, record management, navigation to rural providers, telehealth escalation, emergency-care recommendations, and follow-up. They exclude diagnosis and treatment. Under Section 2(d), the agency must set standards that: (1) require the developer to hold the necessary patent or licensing rights; (2) provide for health data sharing while protecting patient privacy, including electronic health record certification, interoperability, and algorithm transparency; (3) include guardrails against consumer misinformation and against the platform offering services that would be the practice of health care by a professional licensed under Title 34, Code of Alabama 1975; and (4) cover any other consumer-protection standards the director deems necessary. Under Section 2(e), rural health care providers and state health institutions that contract to use the platform would be exempt from Article 5 of Chapter 4 of Title 41 and Chapter 16 of Title 41, Code of Alabama 1975. Section 3 sets an effective date of October 1, 2026.
